About Us

Uffculme is an attractive village, situated close to the Devon/Somerset border in the Culm Valley. The nearest major town in Cullompton, approximately 5 miles to the south-west, whilst the Cathedral City of Exeter is 17 miles in the same direction. This is a rural area and much of the economy is geared to agriculture.

The Village provides facilities including shops, two public houses, church, library and schools and a working woollen mill, which attracts many visitors all year round.

Kent Farm has a quiet position on the edge of the Village and local facilities are within walking distance. The home is built around a courtyard which offers a private secure area for residents use, family entertaining and events eg. cheese and wine afternoon, barbeques etc. There are further gated grounds at the other side of the building which is also accessible to residents whilst also maintaining a safe environment.

Person Centred Care Planning

All residents at Kent Farm have an individual care plan which is devised through using various sources of information eg:  pre admission needs assessment, information from the resident and their family or advocate, information from other health professionals and other organisations involved in their previous care.  The care plans are focused on the persons individual needs and are detailed and extensive.   Risk assessments are extremely important in helping assess individual needs thus ensuring we maintain a safe environment for both carers and residents along with implementing safe care practices.   Care plans are reviewed monthly by the resident and or their families, ourselves and other health professionals where appropriate.  Care plans are signed by the resident, however if they are unable to do so then a significant relative will sign on their behalf.

Nutrition

We believe good nutrition and hydration is crucial to our residents wellbeing.  We therefore monitor food and fluid intake for residents who may have a long term health condition or have difficulty eating and drinking.  Special diets are catered for eg:  diabetics, dialysis patients.  Our food is sourced from local suppliers eg:  local butchers for fresh meat, local farm shop for fresh fruit and vegetables.

Kent Farm provides a varied menu of home cooked meals served from a well equipped and modern kitchen.  Our dining room is located close to the lounge and offers a particular atmosphere.  We offer the choice of three cooked meals a day if required.  Breakfast ranges from a full English cooked breakfast, to porridge, cereals and toast.  For lunch we offer an alternative menu choice each day, including vegetarian choice, together with homemade desserts and homemade cakes and patisseries for teas.

Health Care

At Kent Farm we believe all residents should have access to a GP of their choice and we actively promote residents wellbeing by regular reviews of their healthcare needs which will include visits to their surgery to see their GP or nurse, or GP visits to the home should this be required.  The district nursing service attend at the home weekly as we request, to compliment the care our residents receive should they have a clinical need.

The home advocates for residents and families when required ie. attendance at consultants appointments and follow up tracking of medical care needs.  We arrange transport and extra staffing if required to attend hospital appointments and X Ray appointments.  We offer a high standard of service in ensuring our residents receive all the medical care and follow up they require.  We work closely with various consultants covering a range of medical conditions.
